Wellness Evaluation and Care
At PIVOT, clients will schedule an appointment with our FHNP (Family Health Nurse Practitioner) for a preventative healthcare evaluation, similar to an annual check-up.
During the initial well-care visit, clients can expect the following:

Measurement of weight/height

Calculation of BMI (Body Mass Index)

Evaluation of blood pressure

A series of screening exams

Blood and urine lab panels
During this visit, the patient and provider will set health goals based on the results of these health screenings. Follow-up appointments will be scheduled as needed to monitor progress on the treatment plan set during the initial visit.
**It's important to note that these visits do not substitute for your regularly scheduled primary or specialty care appointments. Instead, our services focus on prevention only. In the event of any new health issues, we will refer you to the appropriate specialist.
